Six-Month Initial Commitment. Membership requires an initial six (6) consecutive month commitment. Monthly payments will be automatically charged to the payment method on file during this initial term.

Automatic Month-to-Month Continuation. After the initial six-month commitment, membership continues on a month-to-month basis until properly canceled.

Cancellation Notice. After the initial six-month commitment has been completed, the member may cancel by providing at least thirty (30) days’ written notice. The member remains responsible for any membership payment scheduled to occur during the 30-day notice period. Upon cancellation, one may not reenroll in the Membership program for 12 months. Any remaining balance must be used within 30 days of the cancellation date.

Written Cancellation. Cancellation requests must be submitted in writing and may be emailed to cosmetics@brynmawrdermatology.com, Verbal cancellation requests are not considered final until received in writing.

Banked Funds. Each successful $99 monthly payment is credited to the member’s account. Banked membership funds do not expire. Upon cancellation, previously paid banked funds remain available to the member, subject to applicable law and practice redemption policies; however, membership-only discounts, perks, promotional pricing, and complimentary benefits end when membership terminates.

Payment Authorization. The member authorizes the practice to automatically charge the payment method on file for the monthly membership fee. The member is responsible for keeping payment information current. Your recurring payment will be withdrawn on the 15th of every month until cancelled.

Failed Payments. If a scheduled payment is declined, membership benefits may be paused until the account is brought current. The practice may contact the member to update payment information and reprocess the payment.

Use of Banked Funds. Banked funds may be applied only to eligible services provided by the practice. Unless otherwise required by law, banked funds have no cash value, are not redeemable for cash, and may not be used to purchase gift cards or medical services/prescriptions.
Non-Transferability. Membership, banked funds, complimentary benefits, and member pricing are for the enrolled member only and may not be transferred, shared, sold, or assigned to another person except where required by applicable law.

Promotions & Discounts. Membership discounts and benefits may not be combined with other promotions, gift-card promotions, special events, manufacturer offers, or discounts unless specifically stated by the practice. The practice will identify when a promotion may be combined with membership benefits.

Complimentary Benefits. Any complimentary annual service or birthday benefit is subject to eligibility, scheduling availability, clinical appropriateness, and the current membership offering. Complimentary benefits have no cash value and are not exchangeable for cash.

Clinical Eligibility. Membership does not guarantee that a particular aesthetic treatment is medically or clinically appropriate. All services remain subject to provider evaluation, informed consent, contraindications, and applicable clinical policies.

Appointments & Cancellation Policies. Membership does not replace the practice’s standard appointment, late-cancellation, no-show, deposit, or treatment policies. Those policies continue to apply.

Changes to Membership. The practice may modify future membership benefits, eligible services, or program terms with advance written notice as required by applicable law. Changes will not reduce banked funds already paid by the member.

No Guarantee of Results. Membership provides access to benefits and prepaid value; it does not guarantee treatment outcomes or specific cosmetic results.
